We are very pleased about your interest in the topic of patient safety. A main objective of patient safety is to avoid and prevent adverse events resulting from errors. Adverse events defined as unintended outcomes of medical care pose serious threats to patient safety. Many adverse events are the result of errors which originate in complex environments based on division of labour. Improvement of patient safety requires learning from errors.
